If called on an empty collection, don't let OEnumerationByIndex.nextElement()
call XIndexAccess.getByIndex() with an invalid index, just like
OEnumerationByName.nextElement() doesn't.

Update javaunohelper's MultiTypeInterfaceContainer.java to use generics, and
fix performance bugs in getContainer() where O(n) iteration over all keys
was being done instead of an O(1) map.ge

where iteration over keys and then n calls to map.get() for the value was

these impact OpenOffice.

The patch for <https://sourceforge.net/p/expat/bugs/539/> is included so
we no longer need a local patch for that. We do need a new patch to
work around the lack of <stdint.h> in MS Visual Studio 9.0.
